‘Wisp of smoke sign" - Spontaneous echogenic contrast in a patient with dilated cardiomyopathy

Abstract
Aim: To highlight the unique appearance of Spontaneous Echo Contrast (SEC) in the Left Ventricle of a patient with Dilated Cardiomyopathy and low ejection fraction

Methods: Image and Video from Transthoracic echocardiography performed to assess cardiac function

Results: 'Wisp of Smoke' pattern of echogenic contrast noted within the left ventricle

Conclusion: While commonly noted in atria rather than ventricles, this echogenic pattern is a marker of low-velocity blood flow and lends itself to a higher probability of thromboembolic events.
